The Community Paediatric Medical Service provides a range of services for young people and their families that are vulnerable due to disease, disability and disadvantage.
These include:

Medical Management
The service is provided for young people under 18. They must be registered with a Sutton GP in the following areas:

Services for children and young people with disabilities and complex health needs

Services to children and young people with SEND. This includes contributing to EHCNAs and annual reviews of EHCPs

Safeguarding and protecting children and young people

Services for Looked After Children. This includes adoption and fostering services
