What is this service?
This is a day service operating Monday to Friday between 9.30 am - 4.00 pm.
Aims of the service:

Enable people to build strong communities for themselves and increase their independence.

Support people to contribute in a positive way to their local community.

Help people to access community and specialist facilities to have healthy, fulfilling and enjoyable lives.
The people receiving the service will be supported to:

Be involved, along with their carers, to develop individual support plans that meet their needs as identified by Social Work Care Plans.

Engage in a range of activities to maximise independence.

Participate in activities within the Inclusion Centre and in collaboration with specialist healthcare providers to address complex individual emotional and physical health needs.

Engage in activities within the community.

Engage in activities that help develop and maintain essential life skills.

Find imaginative ways to make a positive contribution to their local communities to enable people to take on valued roles as active citizens.
Retain a wide range of interests and explore new challenges.
Examples of current activities supported by the team include:
Community access/ambling, Music and dance, Ice skating, Music/sound beam, Tactile sensory, Art, Cookery, Sensory activities, Intensive Interaction, Interactive stories, Rebound therapy, Inclusive cycling, Relaxation and massage, as well as interactive sports including Boccia and Bowling.
Service Cost
The cost of the services is £50.00 per hour of service delivery.
If individuals are attending community activities that have an admission charge the individual is responsible for this cost.
Anyone requiring a meal whilst attending the service must supply their own food.
Referral Process

Referrals can only be made by a London Borough of Sutton Care Manager/Social Worker or an ICB (Integrated Care Board) professional.

The person must have an up-to-date assessment and care plan.

On review any assessment must highlight any perceived changes (+/-) to the level of service being offered.

The Service will look to meet the outcomes as stipulated in the Assessment and Care Plan.

We will work through a risk assessment with associated carers and the care manager making the referral.

The SIT will consider the person’s suitability based on the services ability to adapt to the needs presented.

The service cannot be assumed to be able to offer holiday cover for people who do not normally attend the service.

The service will work with people over a period of time before commencing the service to allow the person to get to know the service, and vice versa, and to build up a complete picture of the support needs.

The service can support people with health care needs that do not require qualified nursing staff.

All referrals will be considered based on meeting the continuing needs of current people within the service.

Once the service has commenced a review will be called after a maximum of six weeks. To review the placement and adjust the package of care/support accordingly
