EHCP Coordinators and the SEND Service

EHCP Coordinators work within Sutton's SEND Service and are responsible for coordinating Education, Health and Care (EHC) needs assessments and maintaining and ceasing to maintain Education, Health and Care Plans (EHCPs).

Each child or young person with an EHCP will have an allocated EHCP Coordinator. As coordinator allocations can change, the SEND Service no longer publishes individual coordinator contact details online. Once an EHCP Coordinator has been allocated to you/your family, you will receive an introductory email from them and an opportunity to speak with them.

What does an EHCP Coordinator do?

An EHCP Coordinator may:

• Coordinate the Education, Health and Care needs assessment process.

• Work with families, education settings and professionals involved in an assessment.

• Maintain and amend Education, Health and Care Plans where required.

• Support the statutory annual review process and action the recommendations arising from it.

• Coordinate phase transfer reviews and transitions between educational settings.

• Identify and name suitable educational settings.

• Act as a point of contact within the SEND Service regarding an EHCP.
