Home from Hospital is an enabling service offering people aged 65+ support immediately after discharge following a stay in hospital, offering a range of practical support to help you

Home from Hospital works with health, social care and community services to support you following a stay in hospital, by helping to build confidence, supporting wellbeing, reducing length of stay at hospital and reducing likelihood of re-admission to hospital.
Your Home from Hospital Care Coordinator will listen to you to understand what matters to you and what you may be concerned about, and will work with you and other health and social care professionals to support your recovery.
The service is delivered over a four-week period, though we can also assist with finding more long-term support at home if this is needed.
The service is free but there are eligibility criteria for accessing the service.
the service is for people aged 65+ who live in Sutton and have little or no informal support network. Please include details in your initial enquiry to help us check if you are eligible to access the service.
If you, a relative or friend are aged 65+ and due to discharge from hospital back to Sutton, get in touch with us for more information.
We aim to respond within 2 working days. Our hours of operation are 9am to 5.30pm, 5 days a week (excludes Sundays and Bank Holidays)
The service is delivered by telephone and in your home.

We deliver training to companies who either want to improve their workplace for deaf colleagues. Also to services who have deaf customers and want to deliver good customer service to all members of the community. £25 per person (minimum 10 people). Contact us now!

We provide accessible peer and wellbeing support sessions for people living in Sutton affected by dementia. Our groups provide information about how to live well with dementia, with guest speakers and topics to chat about over tea and coffee.
