What is this service?

What does Home from Hospital do?

Home from Hospital works with health, social care and community services to support you following a stay in hospital, by helping to build confidence, supporting wellbeing, reducing length of stay at hospital and reducing likelihood of re-admission to hospital.

Your Home from Hospital Care Coordinator will listen to you to understand what matters to you and what you may be concerned about, and will work with you and other health and social care professionals to support your recovery.

How long does the service last?

The service is delivered over a four-week period, though we can also assist with finding more long-term support at home if this is needed.

What we offer?

  • Regular welfare checks for up to 4 Weeks
  • Connecting you with other, long-term support, such as our Community Support Service
  • Emotional support and confidence building
  • Liaison with health and social care services to support your recovery
