Family Hubs bring together the support a family may need, from pregnancy through to young people turning 19 (or 25 if they have SEND).

You will be supported by a Specialist Support Worker to help you improve outcomes for your whole family. We will complete a whole family assessment followed by a plan that will involve lots of other professionals.

We primally take referrals from professionals where they may be worried that without intensive support outcomes for your family may not be good.
We can offer support to all families living in Sutton where there are children living in the family home, a referral is required to access this service which will be triaged by an Early Help Coordinator to identify if we are the right service for you family.
We encourage you to talk to a professional if you are worried about the children in your family. If the professional feels you would benefit from support by a Specialist Support Worker (SSW) they can make a referral on your behalf.

Improving relationships in your family

We will only work with you and your family if you have agreed to support, we work with consent.
Once you have been allocated a SSW they will complete an assessment on all of your family to really understand what is going on for you and what needs to change.
The assessment will be followed with a plan and a meeting with you and any other relevant professionals that can help progress your plan.
There will be ongoing reviews against progress of the plan and once all outcomes have been achieved we will talk with your family about closing. The closure will be celebrated with you and a resilience plan will be created to remind your family of how for you have come and provide you with strategies should similar problems start to happen again.
