As you get older, you may need support to help you live as independently as possible.

Planning for adult life can start before you turn 18. This gives you, your family and professionals time to think about the support you may need in the future.

What age can I be referred to Adult Social Care?

Planning for adulthood can begin from around Year 9 (age 14).

Transition assessments usually take place between the ages of 16 and 18.

Adult Social Care services normally begin when you turn 18.

I am 18 or over and have not received support from Social Care before. How can I get help?

You can contact Adult Social Care directly and ask for an assessment.

A referral can be made by:

  • You

  • Your parent or carer

  • A professional, such as a teacher, support worker or health professional

The Adult Social Care team will talk to you about your needs and decide whether an assessment is needed.

Can I have a Social Care assessment if I have never received support before?

Yes.

You do not need to have received Social Care support as a child to ask for an assessment as an adult.

You can request a Care Needs Assessment. This assessment looks at what care and support you may need as an adult.

The assessment will consider how your needs affect your daily life and whether support may be available.

Should my Social Care assessment be discussed at my annual review?

Yes.

It is a good idea to discuss any assessments, support needs and future plans during your annual review.

This helps everyone involved understand your goals and plan for the support you may need as you move into adulthood.
