The EHC plans should actively monitor young people’s progress towards their outcomes and long-term aspirations.
The EHC plan must be reviewed, as a minimum, every 12 months. The first review must be held within 12 months of the date when the EHC plan was issued. Reviews must focus on the young person’s progress towards achieving the outcomes specified in the EHC plan. They should also be used to consider if the outcomes remain appropriate.
The reviews must be undertaken in partnership with the child and parent or carer. All reviews taking place from Year 9 and onwards must include a focus on preparing for adulthood. This includes employment, independence living and participation in society. Transition planning must be built into the EHC plan. Where relevant this should also include planning for moving between children’s and adults care and health services.
